About the program
The Camden Coalition is a nonprofit that pioneered care management models for people with complex health and social needs and now provides national technical assistance, field-building and training to organizations building complex care programs. It publishes a Complex Care Startup Toolkit and a Blueprint for Complex Care, and has developed a complex care certificate program. Its rigorous randomized evaluation of its own care management model was published in Health Affairs.
