What This Program Does

The Population Health Innovation Institute is MetroHealth's internal engine for designing, testing and scaling value-based care delivery models for a largely safety-net population. It works across three pillars: people (patients, providers and community professionals), data and knowledge (measuring and optimizing care delivery), and relationships (internal experts plus external partners across Northeast Ohio and nationally). The institute focuses on high-risk elderly, chronically ill and uninsured or underinsured residents of Cuyahoga County, and it builds programs such as Red Carpet Care transitions and MetroHealth CarePlus coverage. It partners with payers, community organizations and outside collaborators to pilot and evaluate new care and payment models.
